You may also visit one of DEQ's Public Records Centers located around the state. There are computer workstations available to search, retrieve, view and print records. Our hours of operation are from 8:00 am to 4:30 pm, Monday – Friday. Sometimes all of the Regional Office staff are working in the field, so please be sure to contact a Regional Office before you visit to ensure that someone will be available to assist you.

Use this form to send a request by mail or fax only. To make a request electronically, use the online form.
Certification for Free or Reduced Rate (DEQ Form ISD-0005-02)
Are you a student or a member of the media? These and some other groups qualify for reduced rates for record copies, or in some cases, even free copies of records. Submit this form to firstname.lastname@example.org to qualify for free or reduced rate copies.
